The Intake Process

The facility asks that patients understand that this process will take between two and four hours. Patients must also provide valid identification in the form of a state-issued identification card, a driver’s license, or a United States passport. Patients must also bring a valid insurance card and documentation if they wish to pay for treatment using insurance. All patients must pay their intake fee during their initial appointment.

Unless pregnant or transferring from another program, patients are to arrive at the intake appointment in a state of withdrawal, with between 20 and 24 hours having passed since their most recent use. During the intake appointment, patients will be required to complete the necessary paperwork and answer questions about their personal, medical, and treatment history. They will also be required to complete a physical exam, provide a urine sample for screening, and take a TB test unless proof of a previous, relevant TB test can be produced. Females will also be required to take a pregnancy test. If pregnant, females will need to sign a release of information form to ensure the center can coordinate appropriate care with the relevant OB/GYN.

The center may not be able to provide patients medication on their first day in the facility. This is because all decisions are made on a case-by-case basis by staff physicians.

Vivitrol

    I Would Like to Know more about your facility & processing. Along with when I can get a spot to do this. I’ve been Told that you help people get back on their feet by helping with getting them jobs or out patient. I would like to know how much or what insurance you take.
